House Calls delivers high quality medical care and acts as an interim primary care provider in the most convenient, comfortable environment— the patients’ home.

House Calls accepts most insurance plans, however, some insurance plans may require a referral. Patient should contact his or her individual insurance carrier for more information.
Services Provided
Our services include:
- Diagnosis and treatment of illnesses and conditions
- Review of medical history
- Thorough physical exam
- Comprehensive review of medical history
- Medication evaluation, management and refills
- Order and interpret tests
- Coordination of care to assist patients in accessing community resources
Additional Service Available
If necessary, we also are able to order additional services, including:
- Home health services
- Labs, X-rays and EKG services
- Durable medical equipment
- Social services
- Rehabilitation services
- Hospice services
- Referral to specialists
Limited Mobility Distinction
To be eligible for House Calls, a patient must have a medical condition that requires a significant effort to leave the home, including, but not limited to:
- Complex, chronic disease(s)
- Gait, balance issues or severe mobility impairment requiring cane, walker, wheelchair, crutches or other assistive device
- Terminal (end-stage) illness
- Compromised immune system
- Severe heart disease
- Visually impaired
- Severe disability, including emotional, mental or physical
- Severe memory impairment, such as Alzheimer’s disease, dementia or traumatic brain injury
- Severe pain or weakness
- Recent surgery resulting in complex postoperative needs
- Bedridden or homebound for medical reasons
Additionally, if travel is contraindicated due to a chronic disease, a patient cannot drive or requires a family member to drive due to medical reasons, House Calls may be appropriate.
* A lack of transportation does not qualify a patient for House Calls.
