About Chronic Disease Self-Management
Did you know that more than one in four Americans have multiple (two or more) chronic health conditions? Chronic health conditions may include, but are not limited to: arthritis, diabetes, cancer, heart disease, respiratory conditions, and high blood pressure. When diagnosed with a chronic health condition, the individual must learn how to self-manage the condition. The “It’s All About You” program will teach participants the necessary tools to better cope and care for their diagnosis.
This program is a six week workshop. Through this program, participants will learn to:
- Live with a chronic health condition
- Make smarter decisions related to healthy eating
- Understand the benefits of exercise
- More easily manage medications
Anyone with a chronic health condition or cares for a loved one with a chronic health condition may attend. Participants must be 18 years of age or older.
Participants who complete the program report high satisfaction with the program, improved health outcomes and have decreased emergency room visits and hospitalizations.
Chronic Disease Self-Management Volunteer Leaders Needed
The “It’s All About You” Chronic Disease Self-Management Program is facilitated by trained volunteer leaders. If you have a desire to help others with chronic health conditions, then this program is for you. Volunteer leaders are asked to facilitate at least one six-week workshop on an annual basis. Leader qualifications are as follows:
- Have a chronic health condition
- Comfortable speaking in front of small groups
- Outgoing
- Able to read, write and speak English clearly
- Willing to teach at hours other than traditional business hours, if necessary
- Model healthy behaviours for workshop participants

- A research-based Chronic Disease Self-Management Program (CDSMP) developed by Stanford University
Through this program you will learn to:
- Live with a chronic health condition
- More easily manage your medications

- A lively, fun, interactive research-based 2.5 hour session
- Meet and gain support from others who have similar health conditions
